- To qualify for a medical marijuana recommendation, both new and returning patients must schedule an appointment and provide the following mandatory documents at the time of your visit:
- Documentation from a California licensed health care provider that states your diagnosis. Documents need to be less than 12 months old. Diagnosis documentation can be any of the following:
- Chart notes stating your diagnosis, treatment, and how long you’ve had the problem
- X-Ray/MRI/CSCAN reports. Please do not bring films.
- Letter from your current doctor stating the nature of your illness, how long you’ve had it, and how it’s being treated.
- Valid California identification:
- California Driver’s license
- California I.D. card
- Passport
- Patients under 18 MUST be accompanied by a parent or legal guardian. Parent or legal guardian must also have a valid California I.D.
- Caregivers:
- Must have valid California I.D.
- Must meet criteria defined in Prop 215
- Approval of caregiver certificates is at the discretion of the doctor
- Please make arrangements for child care before your appointment. We appreciate your not bringing small children to your visit.
- A recommendation for medicinal cannabis cannot be made without proper identification and medical documentation, as defined by the California Medical Board Guidelines for Medical Marijuana Physicians.
- If you do not have a doctor, or are uninsured, or haven’t had a check up in the past 12 months, please visit our Local Resources page for a list of local doctors and clinics, so that you can make an appointment and obtain a signed note with your qualifying diagnosis prior to making an appointment with Highland Springs Wellness.
